Abstract
Background. To evaluate the early clinical and functional outcomes of kidney transplantation from a living related donor in patients with end-stage chronic kidney disease and to analyze the impact of intraoperative factors on graft function. Materials and Methods. A retrospective analysis was conducted of data from 10 patients with stage 5 chronic kidney disease who underwent kidney transplantation at the Syzganov National Research Center of surgery. The patients’ mean age was 30.8 ± 10.7 years. Demographic indicators, surgical parameters (cold ischemia time), and early graft functional parameters (daily urine output, creatinine level, estimated glomerular filtration rate) were evaluated. Statistical analysis included descriptive statistics and correlation analysis. Results. In 90% of cases, a kidney transplant was performed from a living related donor. Most patients (80%) had a satisfactory clinical status at discharge. The mean daily urine output was 2,810 ± 1,400 ml, indicating good graft function. A moderate negative correlation between ischemia time and graft function was observed (r = -0.42); however, it did not reach statistical significance (p = 0.22), which may be attributable to the small sample size. In all cases, positive trends in laboratory parameters were observed, including a decrease in creatinine levels and normalization of electrolyte balance. Conclusion. Kidney transplantation from a living related donor demonstrates high efficacy and favorable early clinical outcomes. Precise surgical technique and optimization of ischemia time contribute to adequate graft function in the early postoperative period. The results obtained confirm the need for further studies on larger samples to assess long-term outcomes.
